The skin of your back is thick and there aren't a lot of nerves.

Still hurts a lot. Though due to the nature of the pain it's very easy to go into a trance-like state where the pain doesn't register. Apparantly it brings with it a strong feeling of euphoria.

No, I haven't done it myself. I just know people who have.
