do you have a hard time knowing if someone likes or dislikes something when they say, "that's sick!"

depends on what it is like me farting or me doing a cool jump serve. "that's soo sick". 🙂 then again, i live in so cal, so we got our own slanguage here.
 
online, i automatically assume its something gross/disgusting.

in real life, it all depends on the enunciation and the age range of the person who said it. but yes, in this case i can easily distinguish about the usuage of the phrase.
 
Seems a many of English words when taken to the extreme mean just the opposite depending on how they are used which even confuses the hardest to learn language more 😉
{ie. Bad, Mofo, Hell,}

Then to top things off they start using things like shizzlefritzinguskinkimer 😉

I've never ever heard someone say "that's sick!" and have it mean something positive. Usually for that they say "that's slick!".

and you don't get out much. i have heard "that's sick!" as a positive saying for quite sometime. they aren't saying "slick" Slick.

 
heh, I always thought it was used mainly in reference to sports, but I guess that's just the atmosphere I hear it in the most.

ie. watching my buddy Frenchy deke someone out - "OMG that move was sick, did you see that?!"

yeh, i've seen ppl hold up signs in celebration at sporting events with the phrase on it.
