I don't understand the question. Can you be a little more specific.

HOw do i include the Tuition payment statement in my form? I mean, this mean i cannot use the 1040ez form? Or is it just some useless paper that will do nothign to my taxes?

your second question answered your first question.

Generally, professionals may be able to write off certain tuition expenses, but a normal college student can not.

If your parents claim you as a dependant you can not write off your tuition payments. If you look at the 1040 instructions there is a listing of the criteria you need to meet in order to qualify for this. I think there are 4 things you have to meet.
