Tax question, I never got a 1099 or other docs

I made a couple of bucks doing some work for a guy that I used to work full time for. He hasn't sent me a 1099 or W2 and I have a feeling that he won't. How can I declare this income w/out a document from him? All I have are the check stubs. Is that enough?

I had a similar problem. The company my wife worked for the first half of the year went out of business and she never received a w2.

Thanks for the link. Looks like I will have to go with Form 4852 since the company is kaput and no longer at the old address.
