Sunrocket(assignment for the benefit of creditors)

I filed a dispute last month with my credit card company against Sunrocket and now I recieved this letter for a proof of claim. I guess I am uneasy with fact that they want your social security number. I am wondering if it could be a scam to get info from everyone?

Anyone else recieve a letter?? Please post your thoughts.
 
I received this as well but haven't really sat down to read/fill it out.

Not to threadjack, but on the Sunrocket topic....is anyone that switched to teleblend having problems receiving or making calls to certain #'s? I switched back to Verizon and two people I know who went w teleblend cannot call me. They get a message saying I need to sign up w Teleblend, additionally if you try to call their teleblend # from a cellphone (VZW is the only one tested so far) you get a message the call can't be completed.
 
I don't think I had to give my s.s. to them. I paid with a credit card. I am very leary about the proof of claim form at this time. I hope others will give some insight to this.
