STOP 0100010100x etc. error/-12V exceeds limits

I've been having a little trouble with my computer lately. Currently I have a XP 1700+ Tbred overclocked to 2200+ on an nforce2 2.0 board with a PAL 8045 HS and thermaltake ducting mod with a Vantec Stealth fan on it. I have a couple of panaflo high volume intakes and a bay cooler as an exhaust.

Recently I have had a couple of times when the computer has started up saying it recovered from a serious error and the crash analysis said it had something to do with my CPU. Oddly enough the problem is quite random in occurrence. I also installed the hardware doctor monitoring program and it tells me I'm over my voltage. (13.09 to be exact)

What am I doing wrong? Is there some settings I need to fix? Am I killing my computer?

My temps are 47 on the CPU, 30 on the system.

What make of psu are you running in your case ?
If it's generic then it sounds like it could well be on its the problem, especially as your overclocking with plenty of fans etc, and might want to invest in a good quality psu.
Btw have you tried running your pc at default cpu speed etc and see if you have the same problem ?

Yes, it happens both ways.

I have an Antec 350w PSU, which I thought was pretty high quality.
