Soldier of Fortune 2 help...stuck on train level

Im stuck in the level where you need to get the doctor to the train. I've gotten as far as getting him to the train, but how do I get to the control room? I've managed to get to the point where you go through the hole the in wall and there is a large white tank, but that one guard is there and I dont know what to do about him, please help.

Or throw the knife from far away and land it in his head/neck. Less chance of detection (unless you miss )
