All of my audio file needs are served by CDex and Audacity. Though I don't transcode files all that often.

I always used to use EAC + Lame, but it always had trouble for me with reading cds. Most of the time it would just stop. (not by cd-roms fault, I've tried on multiple pcs.)

Now I use CDex, it does a faster and IMHO, a much better job than EAC.

If a cd is scratched, it will still record it to mp3, but it will just make a note that there was an error. Most of the time the error is not even detecable to my ears.

Exact Audio Copy + Lame is the best. CDex is faster though if you have a flawless CD. However, if there are any scractches on the disc I would definitely recommend using EAC.
