There are a few alternatives, including things like bidpay.com (which has been around for a long time), checks, bartering with pigs and chickens, etc.

Given the prevalence of paypal, it tends to be the "go to" service.

i know some people that use ebay extensivel. people with 4000+ refs that refuse to use paypal
