I have my theme and or idea that I want to create. What are the next few steps of getting everything together? Should I draw out a layout with pen and paper first before dabbling with Photoshop to create the layout?

Normally there is a menu, a contents box, and maybe a header of some sorts with the title of your website. Is that the norm?
 
Start out with a drawing or outline of the basic structure in a photo editor and then playing around with and experimenting with different looks and effects. I find the best looks comes by "accident".
