Smallest atx case?

Do you mean microATX (4-slot motherboard) or ATX (6-7 slot motherboard)?

Ultra MicroFly (see TigerDirect.com) is about the smallest microATX with good cooling that accepts standard ATX power supplies.

Antec NSK4400 is a smallish regular ATX case that ships with a decent (and quiet) PSU with a quiet exhaust fan.
