Shipping to Canada, need help with customs

I need to ship a piece of lab equipment to a company in Canada. I also need to insure it for $1500. UPS is probably the best way to ship it. What forms do I fill out? Also, is the company I am shipping to going to have to pay and tariffs or anything like that?
