I used to have (likely still do) an issue similar, except mine was caused by my manic depression/anxiety. I'm not on any meds, so it wasn't a side effect of those, I found that the solution was getting to better know, trust, and get close to the amazing girl I'm dating. Until I feel secure being with them I have issues like you described, now I have no problems at all. Maybe you've "jumped the gun" a bit and should lay off of it for a while (assumign she understands and all of that.

Hell, most important thing? Talk to her about it (assuming this isn't just a hookup and you are pursuing things with her). If she understands and tries to help you out, even more signs that she's a keeper.

Had to be the condoms. I can last for hours without one and with the right ones. But just last night I had to borrow a "Ultra Pleasure" trojan from someone (Ran out of the magnums ) and I figured they'd be crap. And they were. I found myself loosing my e-rection halfway through. It used to happen to me, back when I was in a college dorm and the roommate wouldn't/couldn't make himself scarce. My solution--I know this sounds lame and not very romantic, but headphones and some loud music usually helped me out. Not pansy-ass makeout music, but the kind of hard rock/metal that I normally don't listen to. Girls don't like getting tangled in the wire though...very not cool.
