Friend has 7 computer systems that are currently setup via a 4 port router, and a 8 port switch. Currently they only use them to connect to the internet and share files between themselves.

They would like to setup a server to handle the following:

Software they use for medical billing everything would be stored on the server, they would like everyone in the network to have access to the client info, create new clients, access existing clients, delete clients, bill clients etc..

What is recommended? Setup as domain controller, and what else?

If it is a medical office they have to conform with Hipaa regulations and should hire a Hipaa specialist consultant.

They are currently running synergy software which they are required to use per hipaa, but they are using a single desktop computer with the clients folder shared across 6 other computers, so if a file gets changed etc. they might only be changing it on there systems and not the computer that currently has all the the software/clients etc on it.

What you'll want to do is contact the company that makes the practice management software they're using. Have them recommend a technician who has completed their training and is familiar with their software. All of the practice management software development companies have training and certification programs for computer technicians, whether it me Medisoft, Dentrix, Eaglesoft, etc. You'll definitely want to use someone who is familiar with the particular software you're using to make sure it gets set up properly in a client-server configuration.
