It happens sometimes at my work. Don't attack anyone just send a memo on the situation.

Like say someone forgot to call a certain party because of an alarm that came in and the process is to call someone the email might look something like this:

Team,

Please remember to call [contact] when [specific] alarms come in as per the process attached. This was not done last night and the customer was not happy. I took care of it and everything is back up now.

With boss CCed. It's kinda passive aggressive, which is sometimes more fun than attacking someone directly. But on serious note if you word it right and don't attack anyone it's more professional, lets the boss know of the situation if they hear about it, and covers your butt if you get the brunt end of someone's actions or lack of.
