Second Monitor Loses Signal

Lately whether I'm using the computer or not, the second monitor will go black and my system will lock up. The taskmanager is at 100% when it locks up and I can do nothing but manually turn off the pc. I recently installed windows xp pro on the system because I got a new hard drive. Also, a couple of weeks after that I moved some cables around in the case to make things look better. Could that have something to do with it? Any ideas would be great, thanks!
 
Okay, I've tried uninstalling different programs that I've installed recently, but it still freezes after a while. Is is possibly a hardware issue?
