SD card safe to remove?

I put my 32gb sd card into my phone, and it said "sd card safe to remove". I tried to mount it and it still came up with that message. Then I put it into an adaptor and then into my laptop to format it. I right clicked on my sd card and clicked format, but nothing happened!

Someone please tell me how to fix this. It would be appreciated.
 
Your phone should have detected the card and format it if needed (on Android at least).
When you put it in your laptop what does it show? I think Windows also asks to format the card if it is not formatted.

If it's a new card try to return it.
