Scythe Mugen 2 Fan Keeps Stopping (Motherboard making beeping noises)

Hey guys, really annoying issue. For the last few months, whenever I boot up my PC I have to open my case (HAF 932) and manually spin the fan on my Scythe Mugen 2 CPU cooler to get it to start up. When it does start up it looks like it's struggling.

The past few days it's started stopping once it had already started. It will go for a few seconds, stop, beeping noise from MOBO ensues, goes for a second, stops, more beeping, goes for a couple second, stops, more beeping. Why does the fan keep stopping? When it dos spin, it's going slow and inconsistent, and seems like the fan is struggling to move, what could be the issue?

First, what mobo do you have? How old is the system?

Does the fan seem to be functional, i.e. does it seem it's purely an issue of the motherboard being unable to supply it with enough/stable power? Does the fan spin normally and smoothly when you spin it manually? Have you tested it attached to the PSU with an adapter?

Have you set any CPU fan control in the BIOS? Does this problem occur regardless of what fan control you set? Can you test the fan in a different motherboard fan header, i.e. chassis fan header?
 
It's a Gigabyte EX58-UD4P mobo. The whole system is about 3 years old, except the graphics card is newer which I recently upgraded.

I have never touched the fan settings in the BIOS. Sometimes when I spin it, it just kind of starts/stops and never gets constant, it feels like it was running out of gas or something and just doesn't have the juice to get going.

Other times I spin it and then it spins as it should, and a day later or so it will just stop again.. or when I turn the system off. Every time I turn it on the fan does not start.
 
I'm guessing the fan bearing is shot.
This. I'd grab one of your case fans, plug it into the CPU fan header, and start up your system to see if it has any issues spinning up. If not, get a new fan, if the case fan also has issues, it's a mobo problem. Fastest way to check.
