Screen tearing even with Vsync enabled?

I recently built a new gaming PC and I'm getting screen tearing in several places; Shogun 2, Cave Story, and even while scrolling when I'm browsing the web using Firefox/Chrome, though it's much worse in Firefox for some reason. Vsync is enabled in the video card settings (its a GTX 560) and that fixed the tearing in Shogun 2, but it still happens in the other two places, though I've played other games like Batman AA without any problems. I've also tried uninstalling, cleaning, and reinstalling the newest drivers, using a different monitor, using a different cable, and using a different video port on the card itself, none of that helped.

Could this be a bad graphics card? I only have one day left in the RMA period with Newegg, so if it's an actual issue with the card I definitely want to get it taken care of before it's too late.

OS is W7 professional 64-bit

edit: after borrowing an ancient radeon x600, Cave Story still tears but Firefox doesn't, so I'm thinking it must be the card's fault.

further edit: i take that back, it still tears it's just not as noticable
