Screen flickers off frequently.. any ideas?

Lately I've been having an annoying problem with my PC - it stops outputting anything onto the monitor completely randomly. It happens frequently coming out of sleep mode, startup, when I'm under a heavy load in a game, and it even chops up my screen during gaming. Here's a link to some of the scenarios I've seen:

I've noticed one of my harddrives clunking and my video card is always on low temperatures.. what could be the problem?
 
have you tried reattaching or completely replacing the monitor cable? do you have a working monitor to test with?
