Edit: Second monitor has been off for about an hour and it just did it again so unless it has something to do with the second monitor simply being plugged in, this is something else causing it.

First off, where did the Computer Help sub-forum go?

At first I thought it was my temps or voltage spikes so I was monitoring all that while playing and nothing is out of the ordinary when it happens and I can't replicate the issue as it seems to just happen randomly. I also noticed when I'm viewing a website, sometimes the webpage will look like it refreshed or like my monitor just refreshed if that makes any sense, but those happen in the blink of an eye whereas the screen going black will take a few seconds to come back on - almost as if the second monitor is forcing my PC to go to sleep but I'm waking it up at the same time. While I'm playing (most frequently happens with Mount & Blade Bannerlord), the game doesn't lag or hiccup at all so I don't believe it's the hardware itself and it only seems to happen when my second monitor is turned on. My main monitor is set at 144hz @ 4K whereas my old monitor is 60hz @ 2K so I don't know if there's some kind of issue there with conflicting refresh rates or resolutions.

I have noticed that when my PC will go to sleep, when I wake it up, if I have any programs running (Steam, Discord, etc.) they will be incredibly small and I have to completely close them and reopen them, otherwise I can't do anything with them. I have no idea if this is related to my new issue, but it's definitely odd, though the last time I used two monitors they were both CRTs so that should give an idea as to how long its been so I could just be missing an obvious setting somewhere.

If it matters, here's my full specs:
PSU - EVGA 1000w 80+ Platinum Supernova
MOBO - ROG Strix Z790-E
Monitor 1 - ASUS ROG Strix XG27UQR UHD IPS 1MS
Monitor 2 - ASUS PB278Q QHD IPS 5MS

Rest of specs are down below in signature.

You don't mention if you are using DP or HDMI. Have you tried swapping cables? Also, do you have G-Sync enabled? I had a similar issue when trying to use G-Sync. Once I turned it off all of my black screens issues went away.

While you put your computer to Sleep some of the games and other type of Active on line connections keep going on the online servers.

When you reconnect there can be a Disparity in what you put to Sleep and what ever is now was going on the Websites that you are connected to and that might result in On Screen false activities.

Try to close your on line activities, then put the conpuier to sleep and see if the same problem occurs.

DP with G-Sync enabled. I'll try out both suggestions but it's a little weird that it's only happening with Bannerlord now.

I'll check this as well. I usually use my second monitor for keeping the wiki up when playing games like Binding of Isaac. I'll try this out as well and see what happens.

Sorry for the incredibly late reply (I honestly forgot I made this post) but disabling G-Sync seems to have fixed the issue.

After I played more games, I noticed the black screen was occurring on other games as well so I started going through the settings to see what linked them and it seems to be an issue with Borderless Fullscreen. I have no idea as to why it happens, but it was especially bad in Rimworld and when I disabled G-Sync, it stopped completely (so far). I also tried running it in windowed mode and enabling G-Sync again and it didn't happen.

The ROG is a 4k 144hz display (DP 1.4). Since Displayport 1.4 is only rated for 4k 120hz at 32.4Gbps it uses Display Stream Compression.
The ROG has hdmi 2.0 and DP 1.4 ports which are both incapable of 4k 144Hz without compression.

So, you could also try running at 120Hz.
