scandisk: "missing media byte"

On my machine, it stopped booting up. I ran some hard drive diagnostics from a floppy, but it doesn't catch any problems. Ran scandisk from boot floppy and it keeps saying the drive is missing it's media byte. Did this before and after low-level and unconditional formats. Is the drive shot?
 
Now it's not coming up with the error, when before, it had. I was able to install Windows 98. I ran scandskw without incident, and scandisk from a boot floppy without problem. even passes surface scans. Seems like this is intermittent? Not sure if I should replace.
