EAX2 If I recall correctly.. I dont think the Audigy supported EAX3, only Audigy 2 does, but I may be wrong.

The Audigy supported EAX3(aka EAX HD) out of the box, it was the big feature that was to sell Audigies. In reguards to what setting you should use, you definately want EAX 3.
 
It's hard to tell, as Creative advertises games as simply using "EAX Advanced HD", which can be either EAX 3 or 4. Supposedly, Jedi Knight: Jedi Academy uses 4, but other than that, I'm not sure.
