sapphire 9500 pro memory question

How do I know if the card is bad? I've gotten screen corruption with standard text when windows starts, and loading certain pictures creates other issues, for example this picture
http://www.dickinson.edu/~skillina/DCP_0478.jpg (800x1205x16M) will load sometimes, or load corrupted in ie, or simply load as "blank" in ie (the image is fully loaded however.)

Using acdsee it loads until I zoom in closer than 50% at which case it brings up an error of "Display driver could not create bitmap"

Then I've had gta:vc crash out on me if I alt tab back into the game bringing video corruption and a hardlocked system.
Then if I've been having problems and have already "used up the video memory" (or so it seems) and try to start anything graphical it will refuse to run, and some things will simply say "not enough video memory"

Heres another example - WinRAR when an archive is opened there are icons on the left side (generally similar to whatever you have associated) in my case all I see is a solid white space where the pictures should be. Rebooting will temporary solve these problems, but I still don't understand why I'm having them in the first place, its as though memory is bad.

Anyways general system specs are as follows:
amd xp 1900+ on ecs k7s5a
sapphire 9500 pro
currently under 98se with cat 3.1s
everything currently running at stock speeds
