Samsung Galaxy S2 question

My dad has had one for a while, but recently he said that he switched the phone on, it took about 30 minutes while it was updating, at which point quite a few things disappeared. I think Android was updated to 4.1.2 (IIRC). Is this likely to have been an update pushed by Samsung (if so, I'm surprised if it's true that there wasn't an option/question about updating first)? He's on pay-as-you-go and he bought the phone himself so it's unlikely (IMO) that his mobile network did it.

So for a few negative points (various widgets/options disappearing, as well as Google News & Weather, which I found a bit odd), there was one positive aspect in that his preferred keyboard layout now works properly with Google apps.

I guess I'm interested for when at some point when I have to replace my phone (HTC). I've had updates (optional), but none have changed the look and feel about how the interface works.

Samsung issues updates but ultimately it's the carrier that does...

I guess a few:

1 - where do the updates come from?
1a - How do updates normally get presented?
2 - has anyone else had an update mangle functionality or aesthetics?
2a - has anyone had Google News & Weather disappear like that?

Mine also is GS2 (Skyrocket), and it's recently updated to Jelly Bean 4.1.2. I found out as your father did some widgets or apps no longer worked such as default Weather widget. However, Google Now has that option so it's not a problem. I believe, any app or widget that is not there anymore, you can replace with other one in the new firmware.
