1Lt and Mrs. firstname lastname (of guy)?

Lieutenant and Mrs. firstname lastname?

1Lt firstname lastname and Mrs. Maiden-Lastname?

some derivation thereof?

As a service member, you don't necessarily need to worry about including rank in there. I wouldn't be offended, but then again, I'm a Reservist. :p

Mr. and Mrs. + guy's full name
or "The [lastname]'s" works as well

Honestly, if you do want to include title of guy and the wife, your question is the same one I have: I have no idea if you just swap out the Mr. and put 1LT, Dr., Sir ("knighted"), or other proper title.

I am going to assume, since you said rank and not position title, he is not in the Navy as an 1LT. In which case, call him Mr. and Mrs. Whatever. He "earned" his rank by literally having to not fuck up for 18 months.

I was in the Air Force, and I will agree, he will probably be offended by you not giving him his proper title, but fuck him. He is a secretary, at best. He will be lucky if in 2 years there are slots for Captain available and he doesn't have to wait another year to actually put it on, because he IS waiting a long time for Major.

How to Address a First Lieutenant:
USA, USAF, or USMC

Envelope, official:
First Lieutenant (full name), USA/USAF/USMC
(Address)

Letter salutation:
Dear Lieutenant (surname):

If you are concerned, probably best to make sure that you are using the correct rank.

Best of luck,
Uno

LT .....

No need to delineate between 1st and 2nd LT. and Col/Lt Col

Same goes for Navy LT/LT JG and Cmdr/Lt Cmdr.

As well as Flag officers

Mail delivery person will not care.
Spouse will not; without the subheading, the higher rank becomes assumed.

If it bugs the lower rank, they have a bur up their ass and should ot advance.

Most formal is

First Lieutenant Theodore Brown
and Mrs. Brown
The theory being that the rank and name are one unit and do not get separated.

I received many though that were separated - following the form

First Lieutenant and Mrs. Theodore Brown

and neither husband nor I were offended, but it is certainly not as formal.

edit: USA/USAF/USMC designations aren't used on social invitations/announcements.

I saw that. My question is as this is a social missive, how do i also address the spouse?

Since it is not an official letter, you can address it as Mr and Mrs. Whatever. He won't get offended, especially if you're not in the military. If you are, and below his rank (or enlisted, which is technically below his rank) he might get upset, but again, fuck him. He is a step above a butter bar and likely still on secretary duty for someone more important. And, if he is an acquisitions officer, he is a failed pilot to boot.
