Running a survey on a website

So I want to run a survey on my website to collect demographic information and to get feedback. I was also thinking of doing a random drawing for giftcards for survey respondants.

Two questions:

1) Does anyone have any experience with offering incentives to do a survey? Were there any problems associated with this?

2) What can I do about minors that would respond? Are there legal problems with sending them a gift card?

Thoughts?

I think you have to be over 13 to win something (or sign up to win something) in the US. If you're worried about strict legalities, I'm pretty sure you need to have your winner complete some kind of "skill test" if the winner is from Canada.

The only problem I can see is getting enough people to respond. If your site has a lot of traffic you're in better shape but it might be nice to advertise that you're randomly selecting a participant to win a prize.

Don't discriminate against miners just because they are dusty little subterranean magic people who till the bowels of the earth for money.

I don't think too many (though some certainly are) will be under 13. I really need the information for feedback purposes, and don't want to get random people coming to the site just for the survey.

FWIW, it gets over 5k visitors/day, so it should not be too hard.
