Resume Question

Long story short - I go by my middle name. I am applying for a job and they want my references. Being a rather nepotistic organization I feel inclined to list my former boss. However, since its been over a year since i worked there I am not sure she will remember my legal first name (they changed everything so that my middle name was on it the second day I worked there stationary, email etc.). Is there a good way to indicate on my resume that they should use my entire name when contacting my references - or even just use my middle and last name?
