Resume cover letter question

So I'm trying to write-up a cover page for my resume, specific to one job posting which contains the contacts name at the bottom. She lists her name without a Ms or Mrs.

Is the proper format for something like this, to refer to her as Ms. <first> <last> or Mrs. <first> <last> or just call her flat-out by her name?

I'm assuming Ms. <first> <last> suits this best.

I just used Ms. <last> and it landed me a phone interview, passed it and now have a in-person interview Thursday.

People are not as anal as you think. Worry more about the content of your letter.
 
I use Ms./Mr. <Lastname> in all my written correspondences, but I always call people by their first name on the phone, just like any business contact would do.
