Resolution Keeps Changing Itself

Is there a way to “lock down” a resolution setting?

I use a 50" HDTV as my monitor. I am running an 8800 GTS card via DVI to HDMI adapter. Vista is the OS.

If my computer restarts, (or sits idle overnight) the screen goes to a lower resolution. I also have to re-position my screen as it cuts off all edges of open windows. I have to run it at 1228X690 so the edges are not cut off. I believe the native resolution is 1280X720.
Right now I am running the 8.17.11.9745 driver but I have had this issue with all drivers.

I also have to constantly resize my scrollbar. It keeps getting narrower on me.
 
My Bro is having a similar issue with a 7900GS with an LCD Monitor. I suspect it's some kind of Driver Bug, like not switching back to Desktop Resolution after exiting a Game or something along those lines.
 
I use a HDTV 42" 1080p for my monitor, use dvi to hdmi, have used vista and now windows 7 as my os. I never had that issue. Are you using nvidia control panel to adjust screen size. I remember having some issues with vista and older drivers but with windows 7 and newer drivers it's been plug and play. Are you using a older HDTV? Does windows recognize the monitor or does it just say generic monitor?
 
The TV is an older 1080i Panasonic TH-50PX500U. It had a VGA connection for a PC. I don't remember why I am not using it.
It does show up as "Generic PnP Monitor".
