Get an old heatsink and fan off a processor, line it up with the holes in the card where the original was held on, and mark the bottom of the heatsink. Drill the holes in the heatsink. Get some little nuts and bolts to fix it on.

This will actually cool better than the standard one, although it will take up an extra slot or two.
 
Remove the old small fan from the heatsink. I believe there are 3 extremely small screws. Then purchase a 50 or 60mm fan. Then you can attach the fan on top of the heatsink using cable ties. Simply use the corners of the heatsink for the cable ties. Then plug the fan's power connector into your PSU (not in the video card).
