When I refund someone do they get every penny back? I don't wanna piss a buyer off if PAYPAL keeps the fees. I would be more then happy to pay the fee's myself..

Paypal does not keep the fees. Click "refund payment" at the bottom of the payment.
 
Fees get refunded. The screen after you select the refund amount, PayPal shows how much that is in principle & fees that are refunded.
