Receipt for Rebate, is shipping slip acceptable?

I know that it has "This is Not an Invoice" across the front of it, but will a copy of it work for a rebate? I made a mistake and didn't print off the receipt when I bought something online (Staples) and the only thing I can find to print off now on the website is order history, and that doesn't have my name or anything on it.

Should I:

-Use the order history printed off?

-Use the slip that came with the package?

-Use your suggestion of something better?

-Kick my butt for not printing the receipt off when I ordered it?
