Rebate Question

I got a few products at Fry's this weekend that have Mail in Rebates. They printed out a "rebate reciept" for each at the register along with my normal reciept. Is this and the UPC all I need to complete the rebate? I am inclined to think so, because each rebate reciept has a little form on the bottom, and both rebates require only a UPC, a rebate form, and a register reciept. So I just fill out these reciepts in the correct fashion and mail them in? Or do I need to get a real form from the rebate websites.

CLIF NOTES: I got a "rebate reciept" printed out at Fry's this weekend. Is that (filled out) and the UPC all I need to get the rebate?
 
on either the rebate form or the rebate reciept... it should tell u extactly what u need. in most cases... all u need is the upc (or a copy), the reciept (or a copy), and the rebate coupon.
