Rebate question for the rebate experts

I purchased a gift for my wife for Christmas (an Oral B electric toothbrush that she wanted). It was supposed to have a $20 rebate and I had a 20% off coupon, so the price was really good. On the store shelves, it said ALL of the 7000 series had a $20 rebate. I selected one of the 7000 series models that best fit our needs. It also happened to come with a free tooth whitening kit that none of the other toothbrushes had. When I got to the register, I specifically asked the clerk and she said the rebate will work with the model I purchased and gave me the rebate form.

I got home and noticed that the toothbrush UPC had an extra letter R at the end of the number list. I checked the rebate form and it had a long list of acceptable UPC codes. The list included the model I purchased except it didn't have the letter R. When the box was opened, there was another box inside (two boxes were inside, the original toothbrush and the tooth whitening kit) with a second UPC - this time without the letter R.

So do I mail in the UPC on the original box that matches the info the receipt, or do I turn in the UPC on the inner box which exactly matches the rebate form? Or am I screwed?

I tried something similar with a toilet paper rebate, I never got anything back on that rebate. I'm worried that sending two would automatically signal that I bought the one with the R which isn't on the rebate form.

dullard = sucks with rebates (I've gotten only one rebate of the three I've filled out in my lifetime).
 
If I'm reading you're statement correctly, the UPC on the inner box matches the number on the rebate form exactly, if so then there's no question. They won't care, or probably know, where you cut it out from, they'll just be happy that it matches what's on the form.

Sometimes a quick follow-up call is needed to get your rebate moving again.
