RE agents: Where to get CAR or PRDS forms?

Title says it all. I'm thinking of purchasing a house without a buying agent, fill out the form and review the disclosures myself ... but I don't know where I can pick up a copy of the CAR or PRDS forms.

Any suggestions?

im an RE agent, and its not a good idea to go into buying a house on your own, save yourself the headaches now and later on by hiring an agent.

ECE: I'm with you but we're sorta in a bind right now. We found our house this Sunday and they're reviewing offers tomorrow, but our RE agent is not able to help us since her dad recently had an operation. She said we should work with the listing agent.

So I figure rather work with the listing agent I might do it myself since I've gone through the process before ... that's the only reason I'm going into this alone. Plus it doesn't seem fair to offer the listing agent a huge chunk of money for so little work.
