Asus 7800GT Silencer - can't get much quieter than this.

Another vote for the Asus. I'll be upgrading my passively cooled X800 XL with that card.

EDIT: You could buy an eVGA one for $275, and fit a NV Silencer or Zalman VF700 on it... Will be cheaper and very nearly as silent...

VF-700 here (well 2 of them for my system).
OR
The Silencer if you case ventilation sux <-meaning it isn't very good
