I'm accepting an internship offer and I'm trying to figure out how to fill in these employment forms for the first time.

I have a direct deposit authorization form and an expense check direct deposit authorization form. The latter is "if applicable." What is an expense check?

They gave me an Arizona state tax form to fill out if applicable. I'm from Ohio, but I'll be working in Arizona for the summer. My bank will still be back in Ohio. Do I need to fill out this form?
 
An "expense check" is often related to traveling expenses.
You work in an office in Town A. You have to travel to Town B to do some work for a couple of days. Say you use your own car to get from Town A to Town B, you fill out some form and send it in to the bookkeeper, then they give you an expense check for X$ per every mile you traveled.

There's other things too, it depends on the company really ...
