Quick Question about an Absent Voter Application

The Application for Abent Voting form for the State of Michigan has a perfed section on the bottom. At the top of the perfed section is written Application to Vote - Poll List. Its got the Ward/Precinct where I am registered and the date of the upcoming general election and then a bar code underneath that. Below that, and slightly to the right are "Ballot No.:" and "Voter No.:". Where do I find the information required for these blocks? The instructions on the form don't say.

The App is similar to this one. Different county, but same form You can see what I am refering to on the bottom of the first page.

Since there is a spot for a signature about the perfed section, I wonder if the perfed section even needs to be filled out.

I called my City Clerk's office, unfortunately, I just got an answering machine.

Anybody know where to get the Ballot #? I am fairly certain the Voter # is the same as a driver's license number.

Elections are run by the counties, not the cities, so the best place to call would be your county's election department, or if you can't get a hold of them, call the Michigan Secretary of State's office. Either one should be able to answer your questions for you.

Even though it doesn't explicitly say as much, I believe that both of those are for the county to fill out when they send you the ballot. I don't think you have to do anything with either of them.

I got thru to the City Clerk's office, they apparently handle the ballots/voter registration, etc. It wasn't necessary to fill out the ballot/voter numbers. Just needed to check the reason, sign it, Print full name, DOB, sign again, mail.
