Question regarding Fry's rebate

I was in Fry's on Black Friday . All the purchase i made have multiple rebate forms to fill out. Those rebate forms look like recipt and they are called rebate recipt. So my question is :
Does that "rebate form & rebate recipt" count both as recipt and rebate form? or i have to make another copy of recipt? Thanks ...
 
The form that Frys prints out for their rebates acts both as a receipt and rebate form (as you surmised).

You still need the UPC codes though.
