question about nuts and washers

I am installing a new cat-back exhaust and have a quick question regarding the lock-washer. No instructions were provided but the distributor says that it should be:

bolt-lock washer-flat washer-flange-gasket-flange-flat washer-nut

but I think it should be:

bolt-flat washer-flange-gasket-flange-flat washer-lock washer-nut

so the lock washer puts pressure on the nut.

What is the correct method?
 
Taking out the flange-gasket-flange interface for clarity...

Bolt head
Flat washer
---
(if the flange is thin, add a flat washer here to distribute the clamping load)
Lock washer
Nut

Although lock washers don't really do much (especially the split ring style). Torque the system tight enough so that the tensile preload resists all the vibrational forces, and a simple nut will hold onto it forever. It's brilliant, really.
