Straight, Cross Over sensing circuits are embedded in all Giga NIC you can always use straight commercial Patches.

Missing, all, no. Most, yes. I have never seen a 1000BaseT NIC that does not support auto MDI/MDI-X. It is not a mandatory feature, though.
