Question about filling out expected salary range and current salary for job prequal form...

i currently make about $65k at my current job that i'm not so excited about

so this hr from some huge software consulting firm contacts me to fill out this questionnaire and it's for a java/c++ related position...

here are two questions that concern me:

what's my current salary range?

what's my salary expectations?

in all honesty i'm willing to take $50k which is a huge cut I think because the job really intrigues me... do you think this is asking for a lot? how should i answer the above two questions? thanks
 
Be honest about your current salary - they can confirm that w/your current employer. As for expectations, contact the HR person to find out what they're offering blind.
