Where do people come up with this stuff? Is it all from one source or is it from a vast majority of people just seeing weird pictures and then captioning them?

Someone creates them and put it on their website. Someone come across that and makes a thread titled "OMG ROFL LOLLERBLADESS.. HAHAHHA... PHUNIEST PIC EVAR!!!!!!!!!!!!!111". People see that, laugh and pass it on to other forums. Somethingawful and Off-topic (their "Owned" pics own) is very well known for this kind of stuff.
 
FARK dot com is the source of many an inspiration...and there are plenty of other more dubious sites with similar 'Photochop' contests that I will not mention here...
