Question about a job application.....

I have an interview tomorrow and I need to fill out this big stupid form to hand in. One of the things that I need to fill out is my work history for the past 5 years. The problem is that I worked in retail 3 years ago and in a factory 3 years ago as well. The retail managers are long gone, and I dont remember the name of my factory manager. What should I put down on the form?

Just put the old manager's names. To your knowledge they still work there.

 
im also supposed to give phone numbers... what should I do? just give the general store #?

What about my manager at oscar mayer? All I remember is her first name...

General store number is fine. Not sure how to handle the last name issue though. Probably just leave it blank or call the place and ask.

 
Just leave the general store number. When or if they do call the store and that person no longer works there, it is common practice if it's a large company for them to contact your former employer's HR department and ask them if so and so was employed, for how long, and if they left on good terms. (To verify with what you put down on the application)
