Problems with Z-560 and Santa Cruz Card

Well I thought it was working properly, but I was clearly mistaken. Here is the situation.

1.) I can't get any sound out of the rear speakers. During the sound check (when I'm assuming sound should be going to all 4 speakers) I get nothing. I have it set to "4 speaker mode", I checked all the fade/balance settings, both on control panel and the master unit (with the sound knob), but I got nothing at all. I've tried with M3D both on and off. Yes, I've checked all the connections. By the way, one time, with a certain file type, I was able to get sound out of one of the rear speakers, but not both. My main concern here is getting them working for my games (Tribes, Tribes 2 and Counter-Strike).

2.) I plugged in my speakers and headphone/mic unit per the following instructions, and I still can't figure out how to get the sound to come out of the headphones. I've tried using the "Output Configuration" to "Headphone Versajack" and that doesn't work. The headphones do work when I plug them into the front control unit.

Below is how I've set up the connections - per the 4 speaker setup.

I can't use the headphones on the control unit (with the sound knob) because they are attached to the Mic, which needs to be plugged into the card itself (if I want to use Roger Wilco).

Any suggestions? I did a search and didn't find anything that would help me. Also, if anybody knows of any software (like a THX test type thing that should test all four speakers and the sub) please let me know.

Update on the rear speakers - I'm now able to get it in one of the rears using M3D, but not both (when playing MP3's). Does that help?

I sure think so. I've got all the speakers plugged in the back as called for.

All the speaker wires have been checked and double checked....

....I am an IDIOT. I checked the wires on the sub, but not the speaker itself. Black one had come loose.

What a great way to spend part of my Saturday - on a problem that wasn't a problem - I was just a dufus.
