Problems with my monitor / video card

I'm not sure where I should start. I have a 19" Futura CRT monitor that I've owned for about 2 years, and an ATI Radeon 9700 Pro that I've owned for 4 months.

A couple weeks ago, I messed around with Windows too much and had to reformat and reinstall everything. Since around then ( * see below), my computer doesn't remember the resolution and refresh rate when I boot up. It just gives me the generic 640 X 480 w/ minimum refresh rate. It also changes the size of the viewing area (not the resolution, the size that you set with the buttons on the monitor) so much that I have to change it every 10 minutes or so.

I've never had this problem before so I'm not sure what to do. I've load the most recent drivers on both my video card and monitor, but it still doesn't work right. If anyone has any questions or thoughts on what I might have forgotten, please help.

*I'm not sure if it ever worked fine after I reinstalled windows, but I do know it worked fine before then.

EDIT: Spelling / Grammar

Thanks for the suggestion, but I've already installed the newest ATI and Futura drivers. Both off their official websites too (www.likomusa.com for the monitor in case you were interested). And while the monitor's drivers don't officially support Windows XP which I am using, I have never had any problems with them before.
