Printing forms on a dot Matrix question??????

Ok basically I have invoices which can use a dot matrix printer to print out the fields on them. Does anyone know any software I can use to "easily" create form fields in the correct locations on the page to print these out?
 
Just use a modern printer and print the entire form, including the filled out fields. Scan one, make it a background image to a form you create in your word processor of choice. Print in color, sorted.
