Depends. IF I'm told business casual and nothing specific to describe it / when in doubt, or not stated otherwise I go with Slacks and button down shirt.
Can get away with Jeans (sometimes) and Polo shirts (more common) at some companies / working at the home office. If you don't care about impressing folks.
Sports Jacket and/or ties for more formal companies or visiting some customers. I'm seeing more and more of this, I think jackets and more formal attire are making a come back but it could just be me and my recent trips.
