98% of the time with 1 (left) hand. I feel much more control with one hand than two (weird but thats how I have got accustomed to).

Anyone driving one-handed all the time drives an automatic. This implies laziness, inability to pilot the vehicle, or a disability that prevents them from exercising total control over their vehicle. In all cases, they are a menace to the road and will be passed by me and my one-handed driving...;)

always two hands, unless the other hand is in use (changing radio stations, browsing for a song on my iPod, holding food/drink, etc)

In the city, always 2 (or if I'm shifting a lot, one on the wheel and one on the stick). On the highway, usually 2. Otherwise one arm gets tired and the car starts pulling to one side:)
